Boundaies and a criteria would be set out for this. For example, 23 out of 25 potential 'Yes's, could mean you have NPD. Questions may differ throughout different institutions for NPD is a fairly recently recognised disorder so there is no strict assessment for it yet, though the questions will be similar.
